Accelerated Degree Graduate Coursework Plan (ADGCP)

Required for accelerated bachelor's/master's students.

Signatures: Student, undergraduate college advisor, undergraduate liaison (if applicable), graduate liaison.

Deadline: Should be turned in when the student is admitted to the accelerated degree program as an undergraduate.

Explains how accelerated degree programs work, including rules for admission, retention, and degree completion.

Add/Drop Form
(not a Graduate College form)

This is a carbon-copy form available from academic departments or from the Office of Enrollment Services in Buchanan Hall.

Signatures: Student, course instructor(s), departmental advisor, Graduate College dean (for graduate students).

Deadline: Refer to the Academic Calendar.

Addition or Change of Program Form

Online addition or change of program application for current OU graduate students or accelerated degree students.

Deadline: Varies by program. See the Graduate Admissions website or contact the academic unit offering the program.

Application for Graduate Credit for Courses Taken During Junior and Senior Years

OU juniors and seniors who have both an OU and overall grade point average of 3.00 or higher may apply to receive graduate credit for a graduate-level course. Accelerated students should complete the Accelerated Degree Graduate Coursework Plan instead of this form.

Deadline: Within the first two weeks of the semester in which the course is taken.

Required for all degree-seeking students. Complete this online form early during the semester you plan to graduate.

Graduate Certificate Program Report (GCPR)

Required for graduate certificate students.

Deadline: No later than the final semester of graduate certificate coursework.

Incomplete Contract (Agreement to Complete a Course after the End of the Term)

This form is available from the Provost's website. It must be submitted to the Office of Academic Records (for Norman campus courses) or PaCS Registration and Records (for OU North America and Europe—formerly Advanced Programs—courses).

Reduced Courseload Request
(not a Graduate College form)

Required for international students who wish to drop below a full courseload. This form is available from the Office of International Student Services.

Request for Evaluation of Transfer Credit
OU North America and Europe (formerly Advanced Programs) or OU Online (formerly Liberal Studies) Studies students only. Other students should consult their department and list any approved transfer credit on the Program of Study or Advisory Conference Report.

Request for Incomplete Grade Extension Beyond One Year

If a student requires more than a year to make up an incomplete grade, this form must be filed with the Graduate College. The original incomplete contract form and revised incomplete contract form should be attached.

This form is customized individually by the Graduate College for each student and sent to the examining committee via email when the exam is authorized.

Signatures: Examining committee.

Deadline: Within one week of the committee's decision, but no later than the final day of the semester.

Advisory Conference Report (ACR)

Required for doctoral students. Outlines plan of study for the degree and establishes doctoral committee membership. See also: Request for Change in Doctoral Advisory Conference Report

Deadline: Must be approved by the Graduate College before the semester the general examination will be taken. Graduate assistants may need to submit it earlier for tuition waiver eligibility.

Entry Form for Dissertation Title on Student's Academic Record

Required for doctoral students. Must be submitted with the final printed dissertation.

Deadline: Although the official deadline to deposit the dissertation is 60 calendar days from the defense, most students will need to deposit much earlier to meet the semester deposit deadline, graduate in the desired semester, and avoid additional enrollment. Refer to the deadlines page for semester deposit deadlines.

General Exam Application for the Doctoral Degree

Required for doctoral students.

Deadline: No later than the end of the second week of the term in which the exam will be taken.

Graduate Assistantship Forms and Documents

This form should be completed by the non-academic hiring department and presented to the chair of the student's academic home department for approval before it is submitted to the Graduate College.

Graduate assistants who are eligible for reduced enrollment in their final semester may be considered eligible for a one-semester exemption from the FICA tax if they declare their final semester. This form may be completed one time only.Web
